Our Chronic & Complex Health Conditions service gives us more time to understand how the different aspects of your health fit together and provide the closer support, accountability and collaboration that more complex cases can require.

Who is this for?

This specialist support may be right for you if you:

  • Have a long-term or complex health condition and need nutrition to form part of your wider care

  • Are managing several health concerns or symptoms at the same time

  • Are underweight or struggling to maintain or gain weight because of a medical condition, poor appetite, digestive difficulties or other health issues

  • Have a current or previous history of an eating disorder or disordered eating, which means dietary changes need to be approached carefully

    Live with ongoing gut, autoimmune, inflammatory, histamine-related or hormone-related health concerns

    Need a more individual approach than standard dietary advice can provide

  • Would benefit from closer support, regular contact and additional accountability

About Nicola

I'm Nicola Shubrook, founder of Urban Wellness and lead practitioner for our Chronic & Complex Health Conditions service.

 

With over 16 years' experience as a registered nutritional therapist and certified functional medicine practitioner, I've worked with many people whose health doesn't fit neatly into one box , whether that's because they're managing several health concerns, struggling to maintain or gain weight, or have a medical or eating disorder history that makes their nutritional needs more complicated.

​

Throughout my career, I’ve worked with many women who have seen multiple specialists, collected numerous diagnoses (or none at all), and still felt unheard. My approach is rooted in compassion, curiosity and collaboration. I take time to understand your full story - medical, emotional and lived experience - so we can make sense of your symptoms together and build a plan that not only genuinely fits your life, but brings about healing and true change to your health.

I'm particularly interested in working with people whose nutritional needs aren't straightforward. These situations often need a more considered approach, and sometimes closer support too.

​

My role is to help you make sense of the different pieces, understand your individual health picture and create an approach that takes your physical health, nutrition and wider circumstances into account. Where appropriate, I can also work collaboratively with your GP, consultant or other members of your healthcare team.

​

What does specialist support look like?

Complex health needs don't always fit neatly into a standard programme. We can offer closer contact and additional accountability, with the level of support tailored to your individual needs.

Clarity Call

Book a 30-minute clarity call so I can better understand your needs, and you can decide if we are the right fit for you.

In-depth Assessment

During our first session, I'll take time to understand your health history, current symptoms, nutrition and overall wellbeing. I'll look at the different factors that may be contributing to your health concerns and identify the areas we want to focus on first.

Personalised plan

After each session, you'll receive a personalised plan with clear, achievable recommendations covering nutrition, lifestyle, supplements and functional testing where appropriate. We'll make changes at a pace that takes your health, circumstances and individual needs into account.

Accountability and on-going support

Between sessions, you'll be able to check in with questions or updates, giving you additional support and accountability as you put your recommendations into practice.

Follow-up sessions

We'll meet regularly to review your progress, talk through how things are going and adapt your plan as needed. With complex health conditions, things can change over time, so your recommendations will evolve with you rather than remaining fixed.

Functional testing

Functional testing can sometimes help us understand particular aspects of your health picture in more detail. We'll always start with a thorough assessment before recommending testing, and only suggest it where we believe the results could genuinely inform your plan. Testing is optional and any additional costs will be discussed with you first.

Supplements

Supplements may be recommended where appropriate, for example to address nutritional gaps or provide additional support alongside your diet. Any recommendations will be based on your individual needs, medications and wider health picture rather than a standard protocol.

Working with your healthcare team

Where appropriate, I can communicate with your GP, consultant, psychiatrist or other members of your healthcare team to support joined-up care and ensure everyone involved has the relevant information.
