Iron and energy: why you're exhausted even when your GP says you're not anaemic

Being told your bloods are "normal" doesn't always mean your iron is where it needs to be for you to actually feel energised. There's a real gap between not being anaemic and having enough iron on board, and that gap is where a lot of unexplained fatigue lives.

Why iron is so tied to energy

 

Every cell in your body relies on iron to produce energy. It's a core part of the process your mitochondria use to turn food into usable fuel, so when iron runs low, that process becomes less efficient across the board, not just in your blood.

 

This is why low iron fatigue often feels different to normal tiredness. It's not solved by a good night's sleep, it's a deeper, heavier kind of exhaustion that doesn't lift.

 

The gap between "not anaemic" and "enough iron"

 

Anaemia is what happens once iron deficiency has progressed far enough to affect your red blood cells, it’s technically ‘late-stage’ by then, but your body starts drawing down on iron stores well before that point, and that draw down can already be affecting your energy, even while your haemoglobin still sits comfortably in range.

 

The marker that shows this earlier is ferritin, your iron storage level. Many standard blood panels don't include it, which is why so many women are told their iron is fine when what's been tested is only the final stage of the picture.

 

Signs low iron might be behind your fatigue

  • Tiredness that doesn't improve with rest

  • Breathlessness on exertion that feels disproportionate

  • Restless legs, especially at night

  • Hair thinning or shedding

  • Reduced exercise tolerance

 

None of these need to be severe to be worth investigating. Low ferritin can affect how you feel long before it shows up as anaemia on standard bloods at all.
